New Participant Referral
You can refer yourself or another person to COSY by phone, website, or completing our
online submission form.
Complete Intake form
COSY understands the importance of knowing and understanding our particpants and their needs. You can request an intake form by phone, website or
clicking here.
Review Consent and Privacy
COSY will
discuss the collection of your information and request your consent to collaborate with relevant care teams.
Complete Service Agreement
Once you have completed your intake process, we will then provide a service agreement and provide you with all of the information required to commence services.
Design your schedule of support
COSY will work with you or your care team to review your funded supports and allocate appropriate funding to our services.
How to apply for the NDIS
If you think you are eligible for the NDIS, you can:
- Contactyour local NDIS Office or local NDIS partner or call 1800 800 110 and ask for an NDIS Access Request Form (PDF 1MB) to be mailed or emailed to you.
- Download and complete the NDIS Access Request Form (PDF 1MB) on your computer and email it to enquiries@ndis.gov.au with supporting information.
- Download the NDIS Access Request Form (PDF 1MB) on your computer, print it out, and mail it to PO Box 700, Canberra, ACT 2601 with supporting information.
- Phone the NDIS on 1800 800 110 to be supported to apply to the NDIS.
